Including subjective data in SOAP notes is essential for capturing a patient's personal experiences and perceptions of their health. However, in the case of an unconscious patient, it is inappropriate to include subjective data because the patient is unable to communicate their feelings, sensations, or thoughts. Subjective data relies on the individual's personal input, which is not obtainable from someone who is unable to respond due to their unconscious state. Without the patient's ability to provide this vital information, any recordings of subjective data would lack validity.

In contrast, scenarios such as a patient providing clear verbal feedback, having family members present to discuss the patient's health, or referencing a pain scale allow for the inclusion of subjective data. In these cases, the patient can articulate their experiences, family members can share insights about the patient's condition, and pain ratings offer a standardized way to gauge subjective pain levels. Thus, an unconscious patient is the only situation where gathering subjective data from the patient is not feasible.
